Implants 4 Life works with many PPO dental plans as an out-of-network provider. Our team can help review available benefits and explain your estimated cost before you decide on treatment.
Using your PPO benefits
Out of network does not automatically mean your insurance cannot help. If your PPO plan includes benefits for out-of-network care, it may contribute toward covered treatment at Implants 4 Life.
Implants 4 Life does not accept Medicare, Medicaid, Medi-Cal, or other government dental programs. We do not offer free implant grants or vouchers.

Know your estimated share
Once your doctor recommends treatment, our team can help you understand three amounts before you decide:
Your treatment fee: the cost of the recommended care.
Your estimated insurance contribution: what your plan may pay toward that care.
Your estimated portion: what you would pay yourself.
An insurance estimate helps you plan, but it is not a guarantee of payment. Your insurer determines eligibility, coverage, exclusions, maximums, and final benefits.

Your insurance benefits are one part of choosing implant care. At Implants 4 Life, the doctors and in-house lab team coordinate the treatment plan and the teeth being made for you, keeping decisions about fit, bite, and appearance connected within one practice.
Insurance questions, answered
- Will insurance cover my dental implants?
- PPO coverage alone does not confirm implant coverage. We can help review the benefits that may apply to your recommended treatment.
- Will you help with insurance claims?
- Yes. When applicable, we can submit claims and help with insurance paperwork.
- What if I don't have dental insurance?
- You can receive care as a private-pay patient. We can explain the recommended treatment and cost, along with third-party financing options that may be available.
- Do you offer payment plans?
- Financing may be available through third-party providers, subject to approval and provider terms. Implants 4 Life does not offer in-house payment plans.
